Recovery after ear surgery needs clear instructions and practical care. Whether you’ve had a tympanoplasty to fix a perforation, mastoid surgery for cholesteatoma, or an ossiculoplasty or stapedectomy for hearing improvement, the first six to eight weeks shape your outcome.

First 48–72 Hours After Ear Surgery

After any type of surgery under general anaesthesia you may feel sleepy. That is why it is not advisable to drive and you will need someone to drive you back home.

Following ear surgery you may experience the following symptoms:

Pain

Mild to moderate pain is common; controlled with prescribed analgesics.

Discharge

There may be slight clear or blood-tinged fluid oozing from the operated ear. Heavy bleeding is not.

Dizziness/Spinning/Nausea:

Dizziness or nausea can occur after ear surgery for 2-3 days. Report persistent or worsening vertigo.

Swelling Below Eyes

There may be slight swelling below eyes (infra orbital area) after ear surgery. Keeping head little propped up, not sleeping too much and walking around helps.

Muffled Hearing

Hearing may be muffled initially. It improves as swelling and packing resolve.

Stiches, Dressings, Ear-Packing

Your ear canal will be packed with an absorbable soft material known as Gelfoam, and a cotton wool smeared in betadine is usually placed on that.  

Cut behind the ear will be sutured with stiches. Your ear will be covered with a mastoid pressure dressing around your head.

Do not blow your nose, do not try to hold your sneeze and keep your mouth open when you sneeze after ear surgery.

Typical Ear Surgery Recovery Timeline

Recovery after ear surgery time line is as follows-

0–3 days: There may be mild pain or discomfort, easily managed by pain killers.
Avoid sleeping on operated ear side to avoid pressure on the ear. Sleep straight or on the un-operated side. You can use contoured memory foam pillow /or U-shaped travel pillow

Keep head elevated when lying down. Avoid bending, heavy lifting, straining and forceful sneezing.

Take soft foods initial 2-3 days after ear surgery, if jaw discomfort makes chewing uncomfortable.

Avoid cold /chilled food items to take precaution for cold/sore throat.

First week (days 4–7): Dressings might be changed; ENT surgeon may remove external packing. Continue keeping ear dry.

Internal ear packing might remain for 1–2 weeks depending on the procedure. Avoid probing the ear.

Sutures behind the ear are usually dissolvable, if non dissolvable then removed around 7–8 days.

Weeks 2–4: Swelling and discomfort decrease. Skin incisions heal.

Internal ear pack is usually out by this time. Brown or blood-soaked pieces of this packing may come out from the ear canal. Do not try to clean it.

Weeks 4–12: Gradual return to normal activity. Final healing from ear surgery and hearing improvement continue up to 3 months in some patients.

Post Ear Surgery Wound And Ear Care

Wound care & keeping the ear dry

Keep the surgery wound and ear dressing dry. Use a plastic ear cover or shower cap when bathing.
Do not insert cotton buds, fingers, or any object into the ear.
If packing falls out or heavy drainage occurs, keep a clean pad over the ear and call the clinic.

How to protect my ear while showering? 

Initial 5-6 days do not wet incision site skin. When allowed to wet the area behind the ear, pat dry gently – do not rub.

Small splashes are usually safe if dressing is waterproofed. Do not allow water to run directly into the operated ear (commonly 4-6 weeks).

After ear packing is removed, you can use fresh clean cotton plug smeared with Vaseline while taking showering and then throw it away after the bath.

Activity, Work and Exercise- What to Avoid and When to Resume

Resuming Activities after ear surgery

No strict bed rest is needed after ear surgery, gentle light walk and doing your normal daily routine is encouraged after ear surgery.

First 1–2 weeks: No heavy lifting (>5–7 kg), no bending or straining, avoid vigorous exercise, avoid driving if you are dizzy or on strong pain medications.

After 2–4 weeks: Light activity may resume as comfort allows. Check with ENT surgeon before returning to contact sports.

When Can I Sleep on My Side After Ear Surgery

Head elevated on pillows, avoid side-sleeping for 3–4 week

How Soon Can I Travel by Air?

For most ear surgeries avoid flying for at least 2 weeks, typically I advise to avoid air travel for at least 4-6 weeks. Follow your ENT surgeon’s specific advice, especially if middle ear is involved.

Swimming after ear surgery

Avoid swimming until surgeon confirms it’s safe (often after 6 weeks). 

Medications & Pain Control After Ear Surgery

Analgesics: Mild pain killers such as Paracetamol and NSAIDs (like ibuprofen) are commonly prescribed.

 

Ear drops: Use 2-3 drops only as prescribed and with correct technique (lie with the operated ear up or use instructions provided).

Children vs Adults- After Ear Surgery Recovery

Children may be more restless; ensure caregivers understand waterproofing, packing care, medication schedules and follow-up.

Return to school typically after7 days of ear surgery, for more extensive surgery follow surgeon’s guidance. Adult need day off from work for 5-7 days.

When to Contact Your Surgeon during Ear Surgery Recovery

Post surgery, call the ENT clinic or go to ER if you notice:

  • Heavy bleeding that soaks dressing.
  • High fever >38.5°C or chills.
  • Increasing, severe pain not controlled by medication.
  • Persistent or worsening facial weakness.
  • Continuous watery or pus-like drainage for >48 hours.
  • Severe, persistent vertigo with vomiting.

Different Ear Surgeries: Recovery

Recovery after Tympanoplasty:

Packing may be placed in the ear canal, Avoid water and nose-blowing for 4 weeks. Expect muffled hearing until packing is removed. There may be some sound or liquid like feeling in the ear.

Recovery after Mastoidectomy

Same as tympanoplasty surgery. Head bandage may be used for 1 week. Delayed return to swimming and flying may be advised.

Recovery after Stapes Surgery

Avoid sleeping on operated side until healed. Avoid forceful sneezing, straining, air travel, heavy weight lifting and vigorous head movements for 4 weeks.

Heavy bleeding, high fever, uncontrolled pain, facial weakness, continuous pus-like drainage, severe vertigo.

Follow up plan after ear surgery

Here is a follow up plan after your ear surgery-
Post op ear care - ear check by an ENT
48–72 hours: Dressing check/ change.

7-8 days: Suture removal / packing status check.

2 weeks: Ear canal packing removal

4–6 weeks: Review of healing and hearing test if needed.

10-12 weeks: Final hearing assessment.

Usually you can return to your work after 7 days. But follow your ENT surgeon’s specific instructions as per the ear surgery performed.

Any heavy bleeding after ear surgery that soaks dressings or does not stop with gentle pressure requires immediate contact.

Mild dizziness can occur for 1-2 days after ear surgery. Persistent or worsened vertigo beyond 48–72 hours should be evaluated.

Swimming or contact sports is usually avoided until your ear surgeon confirms full healing, commonly 6 weeks or longer.

Using a mobile is generally safe after ear surgery. Volume should be kept low to avoid any potential damage to the ear.
