How ENT Doctors Diagnose Ear Pain?

How to diagnose ear pain? Sometimes, it’s not just the ear pain that troubles us — it’s these questions and the anxiety pain brings with it.

Why does my ear hurt?
Is it serious?
Should I see a doctor?
Will I be okay?

If you’ve asked yourself these questions, you’re not alone. Ear pain or earache (medically known as otalgia) is one of the most common reasons people walk into my ENT clinic. They are worried, confused, and often in discomfort.

Let’s walk through how we, as ENT doctors, gently uncover the cause of ear pain and diagnose it to help you find answers and relief or earache.

What happens when you visit an ENT doctor for ear pain? How we proceed to diagnose the cause of ear pain

visiting an ENT specialist to know the cause of for ear pain can be intimidating to some, but it’s usually simple and painless.

Here’s what you can expect during your visit to an ENT doctor for ear pain:

1. We listen first (Symptom History)

Every cause of ear pain has a story to tell. We ask and we listen.

  • When did pain start?
  • Is it sharp, dull, throbbing?
  • Is ear pain constant or does it come and go?
  • Any fever, cold, throat pain, jaw pain while chewing?
  • Any hearing loss, cold, tinnitus, dizziness?

If you’re diabetic or prone to infections, that’s crucial too.

2. We examine the outer ear, ear canal and ear drum

Using 23 years of experience and special tools like:

  • Ear Examination by Otoscope or video otoscope
  • Sometimes an ear microscope
  • Video endoscopic system where we can show you inside of your ear and eardrum
  • Tuning Fork

We ENT specialist check for: 

– Pain on touching outer ear

– Redness or swelling 

– Wax impaction 

– Signs of infection or injury 

– A bulging or perforated eardrum 

Even subtle changes in color or position of the ear drum ( tympanic membrane) tell us where the problem lies.

3. We look beyond the obvious to diagnose the ear pain better

Not all ear pain comes from the ear!
We check your nose, throat, and jaw joints too — because ear pain can come from nearby areas like sinuses or tonsils or TM joints.

Sometimes, the culprit is:

  • TM Joint (jaw) disorders

  • Tooth problems

  • Throat infections

  • Sinus issues

  • Herpes zoster

A normal-looking ear with pain? That’s our clue to look deeper.

How ENT doctors confirm the diagnosis of Ear Pain (if needed):

In most cases, clinical evaluation is enough. But sometimes we may advise:

  • PTA and Impedance (hearing test) – for ear-related hearing issues, or ringing in the ear, or dizziness
  • CT Scan or MRI of the temporal bone or sinuses – if inner ear, skull base issues, or something in the sinuses are suspected.

Otitis media (middle ear infection)

Ear pain, blocked ear, fever, hearing loss, associated cold or sore throat

Bulging/red eardrum, pus

Wax impaction

TM Joint pain

Pain near ear, worse when chewing

Ear looks normal, jaw tender

Dental issue

Throbbing or referred pain

Ear appears fine, dental check needed

How we treat Ear Pain?

Treatment depends on the cause, but may include:

  • Pain relief medication
  • Ear drops or antibiotics (only after checking for eardrum perforation)
  • Cleaning the ear canal gently (if infected with pus or fungus or blocked with accumulated wax)
  • For severe outer ear infections — inserting a medicated wick in the ear canal
  • Sometimes, just reassurance and advice to avoid Q-tips and ear poking

Treatment for TM joint, muscle relaxants, hot fomentation, physiotherapy.

When should you see an ENT specialist for ear pain?

Some signs are red flags and need ear examination and evaluation by an ENT Specialist near you.

  • Diabetes mellitus or low immunity
  • Pain with swelling/redness inside or behind the ear
  • Blisters in the canal/pinna on one side
  • Fever
  • Fluid or pus or water draining from the ear
  • Reduced hearing, dizziness, or ringing in the ear (tinnitus)
  • Persistent or severe ear pain
