What Causes Tinnitus? ENT Explains in Simple Words

causes tinnitus

Ever wondered why your ears ring when everything around you is silent?
Or where that constant buzzing or hissing is coming from? This sound isn’t from outside- it’s called tinnitus.

If you’re hearing it, you’re not alone.

As an ENT doctor, I meet people every day asking this exact question – Doctor, what causes tinnitus and will it ever go away?

In this blog, I’ll explain the most common causes of tinnitus and when you should see an ENT specialist.

TL;DR: Tinnitus is not a disease but a symptom. It can be caused by hearing loss, noise exposure, earwax, ear infections, certain medicines, jaw problems, stress, or other medical conditions. Finding the cause is the first step toward choosing the right treatment.

What Is Tinnitus?

Tinnitus means hearing a sound that isn’t coming from the outside. It could be:

  • Ringing sound
  • Buzzing sound
  • Hissing sound
  • Clicking sound
  • Sound like heartbeat or pulse

Some people hear it in one ear, some in both. Sometimes it’s soft, sometimes loud. It may come and go or it may be constant.

What Causes Tinnitus?

Tinnitus has many possible causes. Sometimes, even after evaluation, no single cause is found.

Hearing loss – Age-Related or General

As we grow older, the tiny cells or sound receptors inside our inner ears can get damaged. These cells send sound signals to the brain. If they’re damaged or disrupted, they send abnormal signal and the brain may “hear” sound even when there’s none.

Loud Noise Exposure

Loud noise exposure can cause tinnitus. Listening to loud music, fireworks, heavy machines, or traffic noise without ear protection can all injure your ears permanently and cause ringing in the ear.

Earwax Buildup

Sometimes, wax accumulates inside the ear it may put pressure on the ear and can cause tinnitus. Removal of the wax safely by an ENT specialist often helps.

Ear Infections or Eustachian Tube Dysfunction

Colds, sinus issues can cause eustachian tube blockage and ear infection and can cause ringing sounds temporarily. Treating the blockage and cold usually helps.

Certain Medications

Some antibiotics, painkillers, and cancer medicines can affect your ears. Always let your doctor know if tinnitus starts after starting a new medicine.

Head or Neck Injuries

Accidents or falls can impact the head or neck and affect hearing nerves and thus cause tinnitus.

Jaw Joint (TMJ) Issues

The Jaw or temporomandibular joint (TMJ) problems can trigger ringing.

Blood Flow Issues (Pulsatile Tinnitus)

If you hear your own heartbeat in your ear, it could be due to high blood pressure or blood vessel issues. Please visit your ENT specialist if you have pulsatile tinnitus.

Your doctor may recommend tests such as MRI or MR angiography if needed.

Stress and Anxiety

Stress can cause temporary spikes in ear ringing. Read our comprehensive guide on how to break the vicious loop between tinnitus and anxiety to start calming your nervous system.

When There’s No Clear Cause: The Brain’s Role in Tinnitus

What If All Your Test Reports Are Normal, But You Still Hear the Ringing?

Tinnitus can still happen even when your hearing tests and scans look perfectly normal. In such cases, the real issue lies in how your brain processes sound, not in the ear itself.

When the brain detects a gap in sound input, it often overcompensates. That means brain may increase its sensitivity to sound, making tinnitus more noticeable (especially in the auditory and emotional circuits of the brain).

That’s why:

  • You may have tinnitus even with normal hearing.
  • It gets worse with stress, anxiety, or silence.

This is what makes tinnitus a neuro-auditory condition, not just an ear problem.

When hearing scans are normal, specialized therapies are required to calm an overactive auditory system. Explore our clinically proven tinnitus relief techniques and sound masking

FAQs - Your Tinnitus Questions Answered by a Doctor

Yes. Many people have more than one contributing factor, such as hearing loss, noise exposure, stress, or TMJ problems. An ENT evaluation helps identify the most likely causes.

If earwax is causing the blockage, wax removal by an ENT doctor can bring relief.

A: Yes. Many people with tinnitus have completely normal reports. In such cases, tinnitus is believed to arise from how the brain processes sound, not from any visible damage in the ear. Even a slight reduction in sound input can cause the brain to generate phantom sounds, leading to persistent ringing or buzzing.

That’s called pulsatile tinnitus. It’s less common and needs a doctor’s checkup.

Stress may not cause it, but it definitely makes it feel louder. Learning to relax helps in perception of loudness and our reaction to tinnitus.

Usually not. But if there are other associated symptoms, or if it pulsatile, see an ENT to rule out any serious condition.
